In a cardiac emergency, proper defib pad placement can save a life. An automated external defibrillator (AED) analyzes heart rhythm. If needed, it delivers a shock. For the shock to work, it needs to travel directly through the heart. That’s why correct pad placement matters.
This guide shows where to place defib pads for adults and children. It uses simple steps and plain language, so responders can feel prepared when seconds count.
Note: This guide is intended for general informational purposes only. Always follow the Instructions for Use provided with your defibrillator and adhere to your organization’s protocols.
Why Defib Pad Placement Matters
AEDs restart the heart or restore a normal heart rhythm by sending an electric shock through the chest. To be effective, the shock needs to pass directly through the heart muscle. Because of this, pad placement is a critical part of any response.
If you place the pads wrong, the shock may not work well. It could even be delayed. That’s why checking placement before activation is so important.
How to Place AED Pads on Adults

For adults, the standard defib pad placement is called anterior-lateral. Place one pad on the upper right chest, just below the collarbone. Place the other on the lower left side of the chest, a few inches below the armpit. This position helps the current travel through the heart in a straight line.
Before applying pads, take a moment to prepare the chest. Make sure the chest is dry and free of obstacles, such as medical patches or thick chest hair. Remove anything that could block contact. By doing so, you improve the chances of an effective shock.
Following these steps allows responders to deliver a strong, uninterrupted shock when it matters most.
Defib Pad Placement in Children Over 55 Pounds
Children weighing more than 55 pounds or generally over 8 years old should use HeartSync adult defibrillation pads. You can also find weight and age guidance in our HeartSync instructions for use.
If the child’s chest is too small to place the pads side by side without overlap, switch to anterior-posterior placement. Put one pad on the center of the chest and the other on the center of the back, between the shoulder blades. This ensures safe spacing and helps the shock reach the heart. DO NOT DELAY THERAPY IF YOU ARE NOT SURE OF EXACT AGE OR WEIGHT.
HeartSync pads include placement diagrams to guide responders.
Important Note About Compatibility
Use HeartSync pediatric pads only with manual defibrillators. They are not compatible with AEDs or use in AED mode. To check compatibility, refer to the defibrillator compatibility chart or contact Nissha Medical Technologies at 1.800.669.6905 or HS.Info@NisshaMedical.com.
Common Mistakes to Avoid in Defib Pad Placement
In a high-pressure situation, small details matter. One common issue is placing the pads too close together. This can block the shock’s path.
Another mistake involves placing pads over patches or devices like pacemakers. These can interrupt the current.
In addition, wet skin, thick chest hair, or tight clothing can prevent the pads from sticking. Before applying, check the chest. Dry the skin and clear away anything in the way. Press each pad firmly to ensure contact.
Correct placement helps increase the chance of a successful rescue.
The Advantage of HeartSync Defib Pads

HeartSync defib pads are designed for ease of use during emergencies. The smaller pad size prevents crowding and makes placement easier. The extra-long leadwires give responders space to work.
Each pad uses strong hydrogel for a lasting hold. A large conductor plate helps spread energy across the chest. As a result, the shock is delivered more safely and effectively.
Together, these features make Nissha defib pads a strong choice for clinical teams.
